What is SSS retirement claim?
The Retirement Claim Application is the Social Security System form a member uses to claim the SSS retirement benefit. It covers the member's details, employment history and dependent children, whether to take the first 18 monthly pensions in advance, the payment account and, for an employed member, the last employer's certificate of separation.
SSS now asks most members to file their retirement claim online through the My.SSS portal. Its retirement benefit page says some claims are filed at an SSS branch or foreign representative office instead, such as those with an outstanding SSS stock investment, privatization, educational or vocational loan, those with dependent children under guardianship, and those where the member is incapacitated. The Retirement Claim Application is listed among the basic documents.
FileIt asks the questions in order, prints everything in capital letters one digit per box, and fills SSS's own three-page form. You sign it, or give your fingerprints, at the SSS branch.

- When a suspended retirement pension can resume because you have stopped working again.
- When SSS tells you your claim has to be filed at a branch.

How to fill out SSS retirement claim, step by step
1 Member's information
Enter your SS number, name, address with postal code, date and place of birth, gender, civil status, TIN and phone number. Dates print as month, day and year in the boxes.
2 Employment history
List your employers, latest first, with the period of employment as MM-YYYY. The form has four rows; use a separate sheet for more.
3 Dependent children
List up to five children below 21, or over 21 and incapacitated, who are unmarried and not employed, with their dates of birth and whether each is legitimate or illegitimate. SSS pays a dependent's pension for each, up to five.
4 Pension options
Say whether you want the first 18 monthly pensions in advance (paid as a discounted lump sum, and only chosen when you first file), and whether you already receive an SSS disability or death pension. For a death pension, give the deceased member's SS number and name.
5 Payment and certification
Choose cash card or ATM/passbook and give the bank details. You certify the information and the statements about your children, and sign at the branch.
6 Certificate of separation
For an employed member, the last employer gives its employer number, name and address, certifies the date you were separated, and signs with the authorised representative's name and designation.
Common mistakes to avoid
- Filing on paper when your claim can be filed online in My.SSS.
- Writing the period of employment as a full date: the boxes take month and year only.
- Listing children who are 21 or over and not incapacitated, married or employed.
- Leaving out the certificate of separation for an employed member.
- Signing before you reach the branch, when SSS asks for fingerprints in front of its employee.
After you fill it out
Print the form from your FileIt vault and take it to the SSS branch with your IDs and the supporting documents SSS lists.
Sign it, or give your right thumb and index fingerprints, in front of the SSS employee. FileIt does not file the claim.
Keep the acknowledgment stub at the bottom of page 1, and the PDF in your vault.
