What is P.F.136 Mauritius passport application?

P.F.136 is the application form for a Mauritius passport, issued by the Passport and Immigration Office of the Republic of Mauritius. It is the full application: you use it for a first passport, for a child's passport, and for a new passport when the old one was lost, damaged or needs replacing for another reason. You must be a citizen of Mauritius to qualify. For the straightforward renewal of a passport that has expired or is full of stamps, the shorter form P.F.136A exists.

The form is a paper application. It asks for the applicant's particulars and a personal description, questions about citizenship and previous passports, the consent of both parents for anyone under 18, a police certificate confirming the applicant's identity and address, and a specimen signature that is reproduced in the passport. It must be completed in block letters with black ink and handed in with original documents and photocopies.

When to use P.F.136 Mauritius passport application

• When applying for a Mauritius passport for the first time, as an adult or for a child.
• After losing a passport: report the loss to the police (or a Mauritian embassy or consulate) first and bring the police report in English or French.
• When a passport is damaged or must be replaced for another reason.
• If you are only renewing a passport that has expired or is full of stamps, the renewal form P.F.136A is designed for that.
• Children under 16 are issued passports valid for five years, and young people from 16 to 18 passports valid for ten years, according to the form's notes.

What you need before you start

• The applicant's birth certificate, as a certified print-out from the Civil Status Office (original and photocopy).
• Two recent identical passport-size photographs, taken less than six months ago, to the size and style described in note 2.
• The National Identity Card for an adult applicant, and the Certificate of Citizenship where applicable.
• For a married person applying in the spouse's name: the marriage certificate; for a divorced person: the marriage certificate and divorce decree.
• For a minor: the parents' birth certificates, the parents' or guardians' identity cards, and any Judge's Order.
• For a lost passport: the certificate or report from the police.
• Details of any previous passport: its number and date of issue.
• Your local address, any address abroad, telephone numbers and email address.
• Your height, eye colour and any visible distinguishing marks.

How to fill out P.F.136 Mauritius passport application, step by step

1 Page 1: name, NIC and initials

The front page repeats the surname, forenames and NIC number in boxes at the top, above the notes. FileIt fills them from your answers in section 1. You can also type your initials in the space after the notes to show you have read them, or leave it for your handwritten initials.

2 (1) Particulars of person in whose name passport to be issued

Give the surname, forenames and maiden name in which the passport is to be issued, the 14-character NIC number, title, sex and date of birth. FileIt works out the age last birthday. Then add the place and country of birth, marital status (with the spouse's name if married), whether your name has changed otherwise than by marriage and, if so, your previous name.

Finally give your contact details: local address, address abroad, home and office telephone numbers and email address. For a child, all of these are the child's details.

3 (2) Personal description

Enter occupation or profession, height, any visible distinguishing mark or peculiarity, and eye colour. In FileIt you type the height in centimetres and it is split into metres and centimetres for the form, for example 172 becomes 1 m 72 cm.

4 (3) Eligibility for a passport

Confirm that the applicant is a citizen of Mauritius and how: by birth, adoption, registration, naturalisation or descent. Then say whether this is the first application for a Mauritius passport. If not, give the previous passport number and date of issue (for a child who travelled on a parent's passport, the parent's passport number) and the reason for replacement: expired, full with stamps, lost, damaged or other. Finally say whether the applicant holds another nationality; if so, the certificate of foreign citizenship must be produced.

5 (4A) Consent of parents or guardian

For an applicant under 18, both parents or the legal guardian authorise the issue of the passport and confirm the child has not lost Mauritian citizenship. FileIt prints the child's name, the parents' names and their NIC or passport numbers. The parents sign or put their right thumbprint by hand, in the presence of a police officer or an officer of the Passport and Immigration Office.

6 (4B) and (5) Certification of consent and police certificate

These parts are completed by officials. In 4B, the officer certifies that the parents signed in their presence. In section 5, a police officer of the locality where the applicant lives certifies the applicant's name, identity and address. Note 4 says this must be done before the application is submitted, so plan a visit to your local police station first.

7 (6) Option for married women

A married woman can choose whether to have her maiden or married name written as an "observation" in her passport. FileIt strikes out the words that don't apply, as the form asks, and you sign this section by hand.

8 (7) Specimen signature

The applicant (or a minor, where able) signs or places a thumbprint inside the box. This is the signature shown in the passport, so it must be done by hand with a 0.6 mm black felt pen and kept within the inner border lines; the form warns that failing to do so invalidates the application.

9 (8) Declaration

Declaration (A) is made by an adult applicant and (B) by a parent or guardian for a minor. Each confirms citizenship and that the documents and statements are true, and warns that false information can lead to prosecution and cancellation of the passport. FileIt ticks the declaration that applies and prints the date; the signature or thumbprint is added by hand.

Common mistakes to avoid

• Submitting the form before the police have certified the applicant's identity and address in section 5.
• Signing outside the inner border of the specimen signature box, which invalidates the application.
• Using ink other than black, or writing names in lower case instead of block letters.
• Bringing photographs that are older than six months, the wrong size, or not identical.
• Forgetting the parents' consent in section 4A, signed in front of an officer, for an applicant under 18.
• Leaving out the police report for a lost passport, or bringing one that is not in English or French.
• Using a birth or marriage certificate that is not a certified print-out from the Civil Status Office with all annotations.

After you fill it out

Print the PDF, then complete the handwritten parts in black ink: the signatures, the specimen signature in section 7 and, for a minor, the parents' signatures in front of an officer. Take the form to your local police station for the section 5 certificate before submitting it.

Hand the completed form in person at the Passport and Immigration Office with the originals and photocopies of the documents listed on page 1. The form notes that the passport will, as far as possible, be issued within four working days after payment. FileIt never submits the application for you.

A copy of the filled form stays in the applicant's folder in your FileIt vault. Once the passport arrives, store a scan of it in the vault too, so you are reminded before it expires and can renew it with P.F.136A.
