What is Passport Annexure F?

Annexure F is Passport Seva's specimen declaration by an applicant who wants a new passport in place of one that was lost or damaged. Below your name, your father's or husband's name and your address, it asks six questions that the passport office uses to look into what happened to the old passport.

You explain how and when the passport was lost or damaged, when and at which police station the FIR was lodged and how many passports you have lost or damaged before; whether you travelled on it and, if so, the flight, date and port of entry into India; whether you used any TR concessions or FTS allowance; where you lived abroad and which passport pages carry the stamps; whether the passport had any objection by the Passport Issuing Authority; and whether you were ever deported at the Government's expense.

FileIt turns your answers into short lines printed under each question on the specimen, fills the residence-abroad table and checks that everything fits. You print the page on plain paper, strike out the S/o, D/o or W/o that doesn't apply and sign it by hand.

  • Not striking out S/o, D/o or W/o where they don't apply.
  • Signing before checking that every answer is true.

After you fill it out

Print the declaration from FileIt on plain paper and sign it by hand.

Take it, with the FIR and your re-issue application documents, to your Passport Seva Kendra appointment. FileIt does not submit anything for you.

Store the FIR and a scan of the declaration in your vault, and add the new passport with its expiry date when it arrives so FileIt can remind you before it expires.

Passport Annexure F: frequently asked questions

Do I need an FIR for a lost passport?

The declaration asks when the FIR was lodged and at which police station, so have the police report ready before you fill it in.

Is Annexure F needed for a damaged passport too?

It covers both: the title is a declaration for a passport in lieu of a lost or damaged passport. For a damaged one there is no FIR to give.

What if I have never lived abroad?

Answer 'No' to question 4. FileIt then prints 'Not applicable: not resident abroad' in the first row of the table.

Can I sign it electronically?

It's a plain-paper declaration signed by hand, so FileIt doesn't offer an e-signature step for it.

What does the declaration commit me to?

You affirm that you will take the utmost care of the new passport, and the form says the Government may take legal action under the Passports Act, 1967 if the lapse is repeated.
