What is LS01 Lost or stolen passport?

If your British passport is lost or stolen while you're abroad, form LS01 tells HM Passport Office so it can cancel the passport. Once it's cancelled it can never be used again, even if you find it. If it turns up, it should be returned to HM Passport Office. The form asks you to send it without delay, to help protect your identity.

LS01 is only for people outside the UK. In the UK you report a lost or stolen passport online at www.gov.uk/report-a-lost-or-stolen-passport. Reporting the loss doesn't get you a new passport either. A replacement is a separate application, and GOV.UK explains how to apply from the country you're in.

Who needs to fill out LS01 Lost or stolen passport?

  • A British passport holder whose passport has been lost or stolen while they're outside the UK.
  • A parent or guardian with parental responsibility reporting the passport of a child under 16.
  • Someone completing the form for an adult who can't do it themselves. Enclose a covering letter explaining why.
  • Not for people in the UK, who report online, and not where there is a parental dispute over who has a child's passport.

When to use LS01 Lost or stolen passport

  • As soon as you realise the passport has gone. The form asks you to return it without delay.
  • After reporting a theft to the local police. The form says passports stolen abroad must be reported to them.
  • Before or alongside applying for a replacement or emergency travel document, which is a separate application.

  • The passport number, if you know it (an old photo or copy of the passport may show it).
  • When, where and how the passport was lost or stolen.

1 01 Details of the lost or stolen passport

The holder's surname, forenames, date of birth, town and country of birth, and the passport number if known, exactly as they appeared on the passport.

2 02 What happened to the passport

Tick lost or stolen, give the date, town or city and country, and tick how it happened. For a loss: while socialising, at home, while moving house, while travelling or other. For a theft: while socialising, mugged or pickpocketed, from home, while travelling or other. If you tick Other, describe what happened.

3 03 Police report

For a stolen passport, say whether you've reported the theft to the police. The form says passports stolen abroad must be reported to the local police. If you have, give the date, the police station and the crime or incident reference number.

4 04 Name of the person completing this form

Only if you're not the passport holder: your surname, forenames and relationship to the holder. For a child under 16 you must have parental responsibility. For an adult, enclose a covering letter explaining clearly why they can't complete the form.

5 05 Contact details

The phone number, email address, current address, country and postcode of the person completing the form. HM Passport Office may need to contact you before it can cancel the passport.

6 06 Completing on behalf of a child (under 16)

Say whether you have parental responsibility, whether anyone else with parental responsibility has the passport you're reporting, and whether everyone with parental responsibility knows you're reporting it.

7 07 Declaration and 08 Where to send the form

Read the declaration: the passport will be cancelled and must never be used again, a replacement needs a separate application, and false information can lead to prosecution. Sign inside the box, date it and print your name. Then send or take the form to the nearest British Embassy, High Commission or Consulate.

Common mistakes to avoid

  • Using LS01 while you're in the UK. Report the loss or theft online instead.
  • Expecting a replacement passport from this form. You need to apply for one separately.
  • Not reporting a theft to the local police first.
  • Leaving the signature or date blank. The form says unsigned or undated forms are returned.
  • Reporting a child's passport without parental responsibility, or when there's a dispute over who has it.
  • Forgetting the covering letter when completing the form for another adult.
  • Using a passport after you've reported it. It's cancelled for good and should be returned to HM Passport Office if found.

Will LS01 get me a new passport?

What if I find my passport after reporting it?

It will already be cancelled and must never be used again. The declaration says it should be returned to HM Passport Office.

Do I need a police report?

If the passport was stolen abroad, the form says the theft must be reported to the local police. Give the date, police station and reference number in section 03.

Can I report my child's lost passport?

Yes, if the child is under 16 and you have parental responsibility. The form shouldn't be used where there's a parental dispute over the child's passport.
