What is Cerfa 12156 - association grant application?

A French association (loi 1901 or Alsace-Moselle) that wants a public grant — for its running costs or for a specific project — fills in the formulaire unique, Cerfa 12156. Service-public.fr says it can be sent by post or electronically to the State, a local authority, a public body, a social-security body or another body running a public administrative service.

The form is ten pages: what you ask for and from whom, the association's identity and legal representative, its approvals and public-utility status, its network and human resources, its budget, then for each project its subject, staff, dates and evaluation with a project budget, an optional equipment request for an event, the legal representative's declarations, and a table of any EU state aid already received.

FileIt turns Cerfa 12156 into a guided form and prints the answers into the official PDF (12156*06, December 2022). It adds up every budget account and the totals, works out the surplus or deficit and the share of the project's income the grant represents, checks the SIRET number, and keeps the project, equipment and state-aid parts optional. The finished PDF is saved in your vault and can be sent for e-signature.

How to fill out Cerfa 12156 - association grant application, step by step

1 Your request

Tick the form of the grant (money, in kind), first request or renewal, running costs and/or projects, annual or multi-year, and the public bodies you send it to with their directorate.

2 1. Identification

The association's name, acronym, website, SIRET and RNA numbers (and, in Alsace-Moselle, the register details), its registered office and correspondence address, its legal representative and the person in charge of the request.

3 2–4. Administration, network, human resources

Approvals with who granted them and when, public-utility recognition, commercial taxes; the networks or federations you belong to and any members that are legal persons; volunteers, civic-service volunteers, employees, subsidised jobs, full-time equivalents, seconded staff and members.

4 5. Budget of the association

Expenses by account (60 purchases to 69 taxes on profits) and income (70 sales, 74 grants by funder, 75 membership fees and gifts, 76–79), in whole euros, plus voluntary contributions in kind. FileIt adds up each account, the totals and the expected surplus or deficit.

5 6. Project

For a project grant: title, objectives, description, beneficiaries, territory, material and human resources, the people working on it, any recruitment, the dates and the evaluation indicators, then the project's budget with indirect charges and own resources, and the grant asked as a share of its total income.

6 Equipment request

Optional: equipment for an event or on a long-term basis, the place, the quantities of each item, delivery, security and guarding.

7 7. Declarations and 7 bis

The legal representative declares the association is up to date with its obligations, subscribes to the republican commitment contract and respects the Charter of reciprocal commitments, states the total public aid received over three years and the amount asked for each year, then dates and signs. 7 bis lists any grants received under EU state-aid rules.

Common mistakes to avoid

  • Giving amounts in cents — the budgets ask for whole euros.
  • A budget that doesn't balance: the surplus or deficit line shows the difference.
  • Contributions in kind used (86) that don't equal the contributions received (87).
  • Forgetting to enclose a RIB, or a mandate when someone other than the legal representative signs.
  • Leaving out the grants asked from other public funders — the form says those figures count as a sworn statement.

After you fill it out

Check every page. The legal representative signs the declarations — on paper, or with FileIt's optional e-signature where the funder accepts an electronic file.

Send it by post or electronically to each public body you ask, with a RIB and the documents the notice n° 51781#04 lists. FileIt doesn't send it.

Within six months of the end of the financial year the grant was for, file a financial report (compte rendu financier) with the body that paid it, as the form reminds you. The application stays in your FileIt vault for next year's renewal.

Yes. FileIt asks the questions part by part, totals both budgets for you and prints the answers into the official PDF, which is saved in your vault.
