What is Kela SV 4e - travel expenses for health care?

Form SV 4e is Kela's English claim for reimbursement of travel expenses (matkakorvaus). It covers trips to medical care, examinations, childbirth and rehabilitation provided through Kela, trips by a family member who takes part in the patient's care, and travel after a traffic or industrial accident. Kela reimburses travel by the lowest-cost means of transport that suits your medical condition and the connections, and the form notes that the copayment for a taxi or ambulance is not reimbursed. Claim within six months of the trip or the payment.

The three pages ask for your details and any personal attendant, your bank account, the purpose of the travel and how the place of treatment was chosen, a table of up to 28 trips (date, provider and addresses, means of transport, kilometres and cost, outward and return trips on separate lines), overnight accommodation costs, the enclosures, and an optional power of attorney so that another person or your employer can collect the reimbursement.

FileIt takes you through SV 4e in plain English, checks your personal identity code and IBAN, totals the trip costs, and prints everything into Kela's own PDF, saved in your vault. As printed on the form, you can file the claim in OmaKela or post it to Kela, PL 10, 00056 KELA. FileIt never sends anything to Kela.

How to fill out Kela SV 4e - travel expenses for health care, step by step

Identity code, name, phone, e-mail, any personal attendant, and the account the reimbursement goes to. Kela gets your address from the population data system.

2 3-4 Purpose and place of treatment

Illness, pregnancy or childbirth; a traffic or industrial accident; Kela rehabilitation; a family member's participation in care; or another reason. Then whether you chose the provider yourself or had a payment or service voucher from the wellbeing services county.

3 5 List of trips

One line per trip, with outward and return trips on separate lines: date, healthcare provider and addresses, means of transport, kilometres and euros. FileIt adds up the total line.

4 6 Overnight accommodation

Who stayed overnight (you, your attendant or a family member), the dates and the cost. Give the reason in section 8 and keep the receipt for six months.

5 7-9 Enclosures, information and signature

Tick what you enclose, add extra information with the section number, then place, date, signature and the signer's printed name and phone.

6 10 Power of attorney

Optional: authorise a person or an employer to collect the reimbursement, with their details and bank account, and sign as the grantor.

Common mistakes to avoid

  • Putting the outward and return trip on one line instead of two.
  • Claiming more than six months after the trip or the payment.
  • Claiming taxi costs without a certificate on the need for a special means of transport.
  • Forgetting the accident report SV 143e when the travel follows a traffic or industrial accident.
  • Leaving out the reason for an overnight stay.
