What is R0820 — Zuschuss zur Krankenversicherung?

Pensioners in Germany who are not compulsorily insured in the pensioners' health insurance (KVdR) pay their health insurance themselves. Under § 106 SGB VI the Deutsche Rentenversicherung can pay a subsidy towards it, and form R0820 (Antrag auf Zuschuss zur Krankenversicherung) is how you ask. The notes say applying in good time matters for when the subsidy starts.

The four-page application asks about you and anyone applying for you, any other statutory pension, how you are insured — a voluntary statutory fund, a German private insurer, or a statutory or private insurer elsewhere in the EU, the EEA or Switzerland — family members whose private insurance costs should count, and whether you are compulsorily insured anywhere. Pensioners voluntarily insured with a German statutory fund such as the AOK, an Ersatzkasse or a BKK.
Pensioners insured with a German private health insurer, the KVB, the PBeaKK or a recognised Solidargemeinschaft.
Pensioners insured voluntarily or privately with an insurer supervised in another EU or EEA country or Switzerland.
Widows, widowers and orphans drawing a survivor's pension who pay their own health insurance.

When to use R0820 — Zuschuss zur Krankenversicherung

Together with your pension application, if you won't be compulsorily insured as a pensioner.
When you move from compulsory to voluntary or private insurance while already drawing a pension.
When you start drawing a second pension, such as a survivor's pension next to an old-age pension.

Your pension insurance number (or, for a survivor's pension, the deceased's).
Details of any other statutory pension: the pension body, its insurance number and since when.
The name of your health insurer and your type of insurance.
Form R0821 or R0822 completed by your insurer, or the date it will follow.
For family members' private insurance: their name, date of birth, relationship and monthly income.
If you are compulsorily insured somewhere: the fund's name and address and the reason.

How to fill out R0820 — Zuschuss zur Krankenversicherung, step by step

1 1 Angaben zur Person

Your names, date of birth, address and phone. The insurance number goes at the top of every page.

2 2 Antragstellung durch andere Personen

Only if someone applies for you — a legal representative, guardian, court-appointed carer or authorised representative. The form asks for the power of attorney or court decision.

3 3 Weiterer Rentenbezug

Whether you draw or have applied for another German statutory pension, and whether a subsidy is already paid with it. The notes explain that several pensions are added up for one subsidy.

4 4.1 to 4.3 Krankenversicherungsschutz

A yes at 4.1 (voluntary statutory fund) skips to 4.5, because your fund reports the details itself. Otherwise 4.2 asks about German private insurance (with R0821) and 4.3 about foreign insurance (with R0822).

5 4.4 Familienangehörige

Whether private insurance costs for a family member should count, with their details, monthly income and whether they draw a pension themselves.

6 4.5 Versicherungspflicht

Whether you are or were compulsorily insured in a German or foreign statutory scheme for the time you claim — the subsidy isn't paid while you are.

7 5 Dokumentenzugang

Optional: large print, Braille, a text CD or a DAISY audio CD, if a disability means you need documents that way.

8 6 Erklärung

You confirm your answers and promise to report changes straight away, such as a new premium, compulsory insurance or a move abroad, and sign.

Common mistakes to avoid

Answering 4.2 and 4.3 after a yes at 4.1 — the form jumps straight to 4.5.
Sending the application without R0821 or R0822 and without saying when it will follow.
Forgetting a second pension, which changes how the subsidy is calculated.
Leaving out compulsory insurance abroad, which stops the subsidy.
Filing one form for a widow and her children — each survivor needs their own.
Not reporting a later change of premium.
