What is G0100 — Rehabilitationsantrag?

When illness or disability threatens your ability to work, the Deutsche Rentenversicherung can pay for rehabilitation — a stay in a rehab clinic or full-day outpatient treatment, rehab for addiction or after cancer, and benefits for working life such as retraining, a vehicle grant or work aids. Form G0100, the Rehabilitationsantrag, is the application for all of them.

Over ten pages it asks what you apply for, whether you need to bring a person you care for, which clinics you would prefer (up to three, optional), your details, job and work pattern, your health insurance and doctor, contributions in Germany and abroad, other benefits, accidents and earlier rehabilitation, and whether someone applies for you. You give your consent to the release of medical information and sign twice. Your doctor's report (Befundbericht S0051) goes with it.

FileIt explains each question in English with the German term, prints your insurance number on every page, places your answers on the official PDF and saves it in your vault. You sign it by hand and send it with your doctor's report.

Common mistakes to avoid

  • Sending the application without the doctor's report S0051.
  • Forgetting the supplementary form named next to the benefit, such as G0110.
  • Letting an authorised representative sign the consent — the form says that isn't enough.
  • Leaving out an accident caused by someone else, which needs form F0870.
  • Describing the last job too vaguely.
  • Signing only one of the two signature lines.

After you fill it out

Print the form and sign it by hand on pages 9 and 10.

Send it with the doctor's report and any supplementary forms to the Deutsche Rentenversicherung, or through your health fund. The Rentenversicherung also takes the application online. FileIt doesn't send it.

Keep the copy in your FileIt vault. If you named no clinic, the Rentenversicherung suggests up to four.

G0100 — Rehabilitationsantrag: frequently asked questions

What is the G0100?

The Deutsche Rentenversicherung's application for benefits to participate (Leistungen zur Teilhabe): medical rehabilitation, rehab for addiction or after cancer, and benefits for working life.

Can I choose the rehab clinic?

Yes. Part 3 lets you name up to three clinics in order of preference. It is optional, and without a choice the Rentenversicherung suggests up to four suitable clinics.

What does my doctor need to fill in?

The form asks you to enclose the Befundbericht (S0051) and the Honorarabrechnung (S0050), which your doctor completes. They are separate forms.

Do I have to give my consent to medical information?

The consent lets the Rentenversicherung get reports from the doctors and clinics you named. You can refuse or withdraw it, but the form explains you then have to supply the records yourself, and a decision may be refused for lack of cooperation.

Can someone apply for me?

Yes, a legal representative, guardian, court-appointed carer or authorised representative, with the power of attorney or court decision enclosed. The consent must still be signed by you or your court-appointed carer.

Does FileIt submit the application?

No. FileIt fills the official PDF and stores it in your vault; you sign it and send it yourself.
