What is OHIP registration 0265-82?

Form 0265-82, Registration for Ontario Health Insurance Coverage, is the Ontario Ministry of Health's application for OHIP, taken to a ServiceOntario centre. If you are new or returning to Ontario you complete sections A, B and C; if you are renewing your photo Health Card you complete sections A and C. The current edition is 0265-82 (2022/10); one PDF carries the form in English and in French.

You present it with original documents from the Ontario Health Coverage Document List — proof of your citizenship or immigration status, of your residence in Ontario and of your identity. If you are over 16 you sign the agreement yourself and your photograph is taken for the Health Card. A parent or legal guardian may sign for applicants under 16. Children under 15½ don't have a photo taken, so they don't need to be present: the parent or guardian brings the child's original documents and this form.

1 A. Personal information

Everyone completes this section: last, first and middle name, sex, date of birth, official language preference, whether you ever had an Ontario Health Number (and what it was), home and work telephone numbers, your mailing address and, if different, your residence address in Ontario with the date you moved there. A P.O. Box, Rural Route or General Delivery mailing address means you must also give your civic address.

2 B. New or returning residents

Where you moved from, when you left that address, when you arrived in Ontario and when you took up permanent residence, and whether you plan to live in Ontario permanently or temporarily. Then: if you moved from another part of Canada, whether a government health plan covered you and your health number there; whether you are a Canadian citizen returning to Canada, an immigrant returning to Canada or a new immigrant; and questions about recently leaving the Canadian Forces, release from a federal penitentiary, being a reservist returning from an out-of-country posting, and being the spouse or dependant of a Regular Force member or of a deployed reservist.

3 C. Agreement

You confirm that you make and intend to keep making Ontario your primary place of residence; that you'll be physically present in Ontario for at least 153 days in any 12-month period; that you won't be absent for more than 30 days within the first 183 days after establishing residency unless an exception on the form applies; and that the information is true. You also agree to tell the Ministry or ServiceOntario within 30 days of a change in your name, address, citizenship or immigration status. Tick who is signing — the applicant, legal guardian, parent or power of attorney — sign next to the X and date it.

Common mistakes to avoid

  • Giving only a P.O. Box, Rural Route or General Delivery address without a civic residence address.
  • Filling in Section B when you are only renewing a photo Health Card, or skipping it when you are new or returning.
  • Arriving without the original documents from the Ontario Health Coverage Document List.
  • A parent signing for an applicant who is 16 or older, who must sign themselves.
  • Using one form for the whole family — each person needs their own.

For a child under 15½, a parent or legal guardian can bring the form and the child's original documents without the child.
