What is SSB Pensioner's Life Declaration (P6)?

People receiving a retirement, disablement or invalidity pension from the Social Security Board of Belize confirm twice a year that they are alive and still entitled to it. The P6 Pensioner's Life Declaration (revised July 2020) is the form for this. SSB says the declarations are due in June and December, and that the monthly pension is temporarily suspended if one isn't handed in during those months.

You fill in your name, social security number, phone, address and email, then mark the one statement that fits you: over 65 on a retirement pension, on a disablement pension, under 65 on a retirement pension, or on an invalidity pension. Under-65 retirees and invalidity pensioners also say whether they are working, and for how long. You then sign in front of an authorised witness, who adds their details, signature and official stamp.

FileIt prepares the form with you, prints it on SSB's own PDF and keeps each year's declarations in your vault. You take it to your witness and hand it in to SSB yourself. FileIt never sends anything to the Board.

How to fill out SSB Pensioner's Life Declaration (P6), step by step

1 Section 1 — your information

Your first, middle and last name, social security number, phone, current living address and email. The pension type at the top of the form is ticked from the statement you choose next.

2 Section 2 — your statement

Mark the one statement that applies to you. If you are under 65 on a retirement pension, or on an invalidity pension, say whether you are employed and give the period of employment. A job you still hold prints as running to 'present'.

3 Section 3 — the witness

If you know who will witness the form, you can type their name, position, address and email now. The witness confirms that you came before them on a given day and signed. Leave that date and both signatures for the day itself.

4 Signing

Take the printed form to your witness, sign and date it in front of them, and let them sign, date and stamp it. Then hand it in to SSB.

Common mistakes to avoid

  • Signing at home before seeing the witness: the witness attests that you signed in front of them.
  • Using a witness who isn't on the form's list, or forgetting their official stamp.
  • Marking two statements, or the wrong pension type.
  • Leaving out the employment period when you are working.
  • Handing it in outside June or December and having the pension suspended.

After you fill it out

Hand the stamped declaration in to SSB, at a branch or through the channel your branch accepts, before the end of June or December.

FileIt keeps each declaration in your vault, so you can see when you last handed one in.

The official-use block at the bottom is filled in by SSB staff.
