What is SSB Funeral Grant claim (FG.1)?
When someone covered by Belize's Social Security scheme dies, the Social Security Board can pay a funeral grant to the person who paid for the funeral or who is responsible for paying it. Form FG.1 (August 2013) is how you claim it. The same form is used whether the person who died was the insured person, their spouse or common-law partner, or a dependent child.
Part I asks about the person who died: their name and social security number as on the registration card, dates of birth and death, occupation, the certified cause of death, their last employer, whether they were receiving a benefit and whether the death came from an accident at work. Part II is about you as the claimant, and whether you have already paid for the funeral or are liable to pay. Part III is a checklist of the documents you hand in with the claim.
FileIt turns the form into plain questions, prints your answers on the Board's own PDF and keeps the finished claim in your vault, next to the death certificate and receipts if you store them there too. You sign it and hand it in to SSB yourself. FileIt never sends anything to the Social Security Board.

How to fill out SSB Funeral Grant claim (FG.1), step by step
1 Part I — the deceased person
Enter the name and social security number as on their registration card, the dates of birth and death, occupation and up to two lines for the certified cause of death. Then the last employer or business and its address, whether they were receiving a Social Security benefit (and which one), and whether the death was caused by an accident at work.
2 Part II — you, the claimant
Your name as on your own registration card, your social security number, date of birth, address, email and phone. Then say whether you are claiming in respect of your spouse, your dependent child or the deceased insured person, and whether you have paid for the funeral or are liable to pay.
3 Part III — documents
Tick each document you are handing in. The death certificate is needed for every claim, with the receipts if you paid or the invoice if you are liable. Add the child or spouse documents that apply to your claim.
4 Declaration
You declare that the information is true to the best of your knowledge, print your full name and date it. If you cannot sign, someone may sign for you and should say on the form that they did so.
Common mistakes to avoid
- Waiting longer than six months without attaching a note explaining the delay.
- Handing in photocopies when the form asks for the original death certificate, receipts or invoice.
- Receipts or an invoice in someone else's name. They should be in the name of the person claiming.
- Spelling the deceased person's name differently from their registration card.
- Ticking both 'I have paid' and 'I am liable'. It is one or the other.
- Leaving out the marriage certificate or JP declaration when claiming for a spouse.
After you fill it out
Check the printed claim, sign the declaration and take it to SSB with the original documents. The official-use section on page 2, where SSB records its decision, is completed by the Board.
Keep copies of the receipts or invoice for yourself before handing in the originals.
FileIt keeps the completed claim in your vault, so you have a record of what you handed in and when.
