What is Parents' authorisation for a minor's passport (Polícia Federal)?

When a child or adolescent under 18 applies for a Brazilian passport, the Federal Police (Polícia Federal) needs the parents' authorisation. The 'Formulário padrão de autorização de expedição de passaporte para menor', based on article 27 of Decree 5.978/2006, is the standard page for it: each parent identifies themselves, the form states the authorisation, and the child's details follow.

The form tells you to complete it before the appointment at the passport post (Posto de Emissão de Passaporte), but the parents sign it there, in front of the officer. If one parent cannot come, that parent's signature must be recognised by a notary beforehand (reconhecimento de firma, by likeness or by authenticity), or the absence is handled as described on the Federal Police's 'Obter Passaporte' service page. The form also reminds parents that the authorisation can only be revoked by both of them or by a court, and that revoking it cancels the passport at once.

FileIt asks for each detail in plain English with the Portuguese wording alongside, checks both CPFs and the child's age, and places every answer neatly on the dotted lines of the Federal Police's own PDF. The finished page is saved in your child's vault folder. You print it and take it to the appointment — FileIt never submits anything to the Federal Police.

Who needs to fill out Parents' authorisation for a minor's passport (Polícia Federal)?

  • Parents getting a first passport for a baby, child or teenager.
  • Parents renewing a child's expired or full passport.
  • A parent who will attend the appointment alone and needs the other parent's notarised signature on the same page.
  • Families who are preparing documents ahead of a trip abroad with their children.
  • Grandparents or relatives helping parents gather the paperwork before the appointment.

When to use Parents' authorisation for a minor's passport (Polícia Federal)

  • Before any passport appointment for someone under 18 — fill it in at home and bring it along.
  • Well before the appointment if one parent will be absent, so their signature can be recognised at a notary in time.
  • When the Federal Police asks for the standard authorisation instead of the parents signing a handwritten statement.

What you need before you start

  • Each parent's identity document (identity card, driving licence, professional card or passport) with its number, issuing body and date of issue.
  • Each parent's CPF, a phone number with area code (DDD) and full address.
  • The child's full name, sex and date of birth, as on the birth certificate.
  • The child's place of birth: the city and state (UF), or the city and country if born abroad.
  • The city and date on which you will sign at the passport post.

What’s on Parents' authorisation for a minor's passport (Polícia Federal)

The 2026 edition has 1 page. FileIt asks for it in 4 parts:

  1. First parent (Eu …)Parent
  2. Second parent (… e Eu …)Parent
  3. The child (menor)Parent
  4. Place and date (local e data)Parent

How to fill out Parents' authorisation for a minor's passport (Polícia Federal), step by step

1 First parent

'Eu …': your full name, the number of the identity document you will show, who issued it and when, your CPF, a contact phone with the area code in the brackets, and your address. FileIt checks the CPF's two check digits.

2 Second parent

The same details for the other parent, in the second block ('… e Eu …'). Normally both parents fill this in; if one cannot attend, their signature must be notarised before the appointment.

3 The authorisation

'AUTORIZO / AUTORIZAMOS a expedição de passaporte': you authorise the passport for your son or daughter. You also confirm you understand that the authorisation can only be revoked by both parents or by a court decision, and that revoking it cancels the passport immediately and irreversibly.

4 The child

The child's full name, sex, date of birth and place of birth ('natural de', city and UF). FileIt prints the date in the form's day | month | year slots.

5 Place, date and signatures

The city and date of signing. The two signature lines stay blank: each parent signs by hand at the passport post, in front of the officer.

Common mistakes to avoid

  • Signing at home before the appointment — the form says the parents must sign in front of the officer.
  • Turning up without the absent parent's signature recognised at a notary.
  • Giving a document number for an ID you won't bring to the appointment.
  • Mistyping a CPF; FileIt verifies the check digits, but compare it with the card too.
  • Leaving the area code out of the phone number.
  • Writing the child's name differently from the birth certificate.

After you fill it out

Print the page and read it through. If one parent will not be at the appointment, that parent signs on their line and has the signature recognised at a notary (cartório) first.

Take the form to the passport appointment with the child's and parents' documents. The parents sign in front of the Federal Police officer, who keeps it with the application.

FileIt keeps the completed form in the child's vault folder, next to the passport once you add it, so the passport's expiry can be tracked and you are reminded well before it runs out.

Parents' authorisation for a minor's passport (Polícia Federal): frequently asked questions

Do both parents have to go to the passport appointment?

The form says the parents sign in front of the officer. If one of them cannot attend, that parent's signature must be recognised by a notary beforehand, or the absence is handled as the Federal Police's 'Obter Passaporte' page explains.

Can I sign this form electronically?

No. It is signed by hand at the passport post or, for an absent parent, before a notary, so FileIt does not offer an electronic signature for it.

Is this the same as the travel authorisation for a child?

No. This form authorises the passport itself. Permission for a child to travel abroad without one or both parents is a separate matter, such as the CNJ standard travel authorisation or an authorisation included in the passport.

Can the authorisation be withdrawn later?

The form says it can only be revoked at the request of both parents or by a court decision, and that revoking it cancels the child's passport immediately and irreversibly.

Can I fill it in online?

Yes, with FileIt: you answer the questions online and get the Federal Police's own PDF back with your answers printed in. You still print it and take it to the appointment yourself.

What if my child was born abroad?

Write the city in the 'natural de' line and leave the state blank, adding the country to the city if it helps; FileIt prints only what you give.
