What is FAMIWAL — birth allowance (modèle E)?

Families in Wallonia receive a one-off birth allowance (allocation de naissance) for each child, paid by their family allowance fund. FAMIWAL, the Caisse publique wallonne d'allocations familiales, uses this form, modèle E, version V16 of October 2024. The mother applies. She can do so before the birth, at the earliest four months before the expected date, or wait until the child is born.

Annex 1 asks for the mother's details and those of the father or co-mother: name, date of birth, address, phone, email and national register number. It also asks whether this is each parent's first child, and about either parent's situation, such as a disability, a long illness, work for an international organisation or abroad, or benefits from another country. Before the birth, the doctor or midwife completes annex 2, a short pregnancy certificate with the expected date and the number of babies. Annex 3 gives the bank account for payment, and annex 4 asks to affiliate with FAMIWAL if you aren't with a fund yet.

FileIt explains each part in English with the French wording alongside. It asks for the mother's details once and prints them on every annex, checks the national register number and IBAN, and writes everything on the dotted lines of FAMIWAL's own PDF, with dates in the Belgian DD/MM/YYYY format. The finished form is saved in your vault. FileIt doesn't send it. You and your doctor or midwife sign, and you post or email it to FAMIWAL.

  • Mothers expecting twins, triplets or more. The doctor's certificate says how many.

When to use FAMIWAL — birth allowance (modèle E)

  • From the 6th month of pregnancy. The form says you can apply at the earliest 4 months before the expected date of birth.

  • Together with annex 4 if you aren't affiliated with any family allowance fund yet.
  • With annex 3 if you want the allowance paid into your account instead of by postal cheque.

What you need before you start

  • The mother's and the other parent's names, dates of birth, addresses, phone numbers and emails.
  • Both parents' national register numbers, from the back of the Belgian identity card.
  • Whether this is each parent's first child, counting stillborn children and those who died after 180 days of pregnancy.
  • Before the birth: your doctor or midwife, who completes and stamps the certificate in annex 2.
  • After the birth: the original certificate for the birth allowance that the municipality gave you when you registered the birth (step 5 of the form's instructions).
  • The IBAN and BIC of a current account in your name, or held jointly with your partner.

How to fill out FAMIWAL — birth allowance (modèle E), step by step

1 1. and 2. The parents

Give the mother's details in the left-hand column: name, date of birth, address, phone, email and national register number. The father or co-mother goes in the right-hand column. The national register number prints one digit per dash, in the form's 6 + 3 + 2 groups.

2 3. and 4. First child?

Say whether this is the mother's first child, and whether it is the father's or co-mother's first. The form's note says to count stillborn children and children who died after 180 days of pregnancy.

3 5. and 6. Situation

Tick what applies to the mother and to her spouse or partner: a disability of at least 66% (with the date and the institution), illness for at least six months, work for an international organisation or abroad, or social benefits from another country.

4 Annex 2: the pregnancy certificate

Only if the baby isn't born yet. The doctor or midwife gives their name, confirms the mother is at least five months pregnant, and adds the expected date of birth and whether she expects one child, twins or triplets. They date, sign and stamp it. FileIt fills in the mother's name and date of birth for them.

5 Annex 3: the bank account

Give the IBAN and BIC of a current account, in your name alone or jointly with your partner. Without it, the allowance comes by postal cheque.

6 Annex 4: affiliation, and signing

If you aren't with FAMIWAL yet, tick your current fund (Camille, Parentia, Infino, Kidslife or another) and note the 24-month rule. FileIt dates annexes 1, 3 and 4. You sign each one by hand.

Common mistakes to avoid

  • Applying too early. The form says the earliest is 4 months before the expected date of birth, and payment comes at the earliest 2 months before.
  • Sending annex 2 after the birth. It isn't needed then.
  • Forgetting to count a stillborn child, or one who died after 180 days of pregnancy, when answering 'first child?'.
  • Leaving out the doctor's or midwife's stamp. The certificate asks for a signature and stamp (signature et cachet).
  • Giving a savings account or someone else's account. Payment goes only to a current account in your name or a joint one.

After you fill it out

Print the PDF. Have your doctor or midwife sign and stamp annex 2 if the baby isn't born yet, then sign annexes 1, 3 and 4 yourself.

Post them to FAMIWAL, BP 80 000, Ville basse, B-6000 Charleroi, or email them to [email protected]. FileIt doesn't send anything.

FAMIWAL pays the birth allowance at the earliest two months before the expected date, by bank transfer or postal cheque.
