What is PS 6.4 ANSES power of attorney (carta poder)?

Formulario PS 6.4, the carta poder, is the power of attorney ANSES uses for social security matters. The benefit holder (poderdante) names a representative (apoderado) and ticks what the power covers: carrying out procedures (para tramitar), such as applying for a pension, or collecting the benefit (para percibir). A separate block at the foot of the page lets the holder authorise a bank to collect their pay.

Who can be named depends on Law 17.040, summarised on the back of the form. For procedures, the representative can be a spouse, a relative by blood up to the fourth degree or by marriage up to the second, or a lawyer or procurator. For collecting, the list widens to public bodies, banks, registered mutual and social-assistance institutions, the directors of a hospital or care home where the holder is staying, and — with a medical certificate showing the holder cannot get about — any capable person. A competent authority certifies the identities and signatures: a pension, judicial, police or consular authority, a notary, or the director of the hospital or similar institution where the holder is.

FileIt asks for the holder's and the representative's details in plain English, turns them into the codes the form uses (document type, province, sex, nationality, marital status), splits the benefit number into its boxes and checks CUILs. You get ANSES's own PDF back, saved in your vault, to print, sign and certify. FileIt never submits anything to ANSES.

How to fill out PS 6.4 ANSES power of attorney (carta poder), step by step

1 Expediente and Beneficio

The file number (agency, CUIL/CUIT, procedure and sequence) if the procedure already has one, and the benefit number split into Ex-caja, T, Número, C and DV. The grey Código de Área box is for ANSES.

2 Quien suscribe — the holder

Name, document type and number, the province of issue (only for a Cédula de Identidad), sex (M/F), nationality (A for Argentine, E for foreign) and full address with the province code from the table on the back.

3 Otorga poder — the representative

Tick to carry out procedures or to collect, and say which procedure. Then the representative's name (or the entity's), professional registration number for a lawyer, CUIL or CUIT, sex, nationality, marital status (1 to 7), date of birth, relationship and address. The grey Cód. Repres., Fecha Alta and Fecha Venc. boxes are left for ANSES.

4 Signatures and certification

The representative and the holder sign, and the certifying authority confirms that the details match the identity documents and that the signatures were made in their presence, adding the place, date, signature and stamp.

5 Power to collect through a bank

Optional. Name the bank and sign with the place and date; the power stays in force until you present an express revocation to ANSES, by public deed or a new carta poder. The bank completes its own declaration, stamp and signature.

Common mistakes to avoid

  • Naming someone the law doesn't allow for that power — for procedures, only close relatives, a spouse, or a lawyer or procurator.
  • Signing before the certifying authority sees you do it; the certification says the signatures were made in their presence.
  • Letting the certification lapse: 30 days for collecting and 120 days for procedures.
  • Using LM or LF for a DNI of 10,000,000 or more, or mixing them up (LM is male, LF female).
  • Filling in the grey boxes, which are for ANSES.
  • Forgetting the last payment slip when the representative goes to ANSES.

After you fill it out

Print the form, then sign it in front of the certifying authority with the representative; they check both identity documents and certify the signatures.

The representative takes the certified carta poder to ANSES with the documents listed on the back: the holder's identity document and last payment slip. For a bank power, the bank also signs and stamps its part.

FileIt keeps the completed form in the holder's vault folder. Note the certification date somewhere you will see it: the power must be used within 30 or 120 days, and you may need a new one after that.
