What is the prenuptial agreement questionnaire form template?
Prenup meetings go faster, and cost less, when the lawyer already knows what the client owns, what they owe, and what they'd like the agreement to say. But that's a lot to cover in a first meeting, and some of it — a business valuation, an expected inheritance — takes time to look up.
This questionnaire is filled in by each partner separately, for their own lawyer. It starts with personal details, the wedding date, the partner's name and lawyer, and any earlier marriage or children. Then it works through what the client owns: real estate, cash, investments, retirement accounts, vehicles, other assets and expected inheritances or trusts.
Answering yes to "Do you own all or part of a business?" opens a full set of ownership questions: name, type, percentage owned, estimated value and how it was arrived at, when it started, any shareholder agreement and whether the partner will be involved. After debts and income, the client says what they'd like for separate and shared property, support and a family home. Each questionnaire is filed as a PDF in your FileIt vault.

2 What you own
Real estate (a yes asks for each property's location, ownership, value and mortgage), totals for cash, investments, retirement and vehicles, other significant assets, and any expected inheritance or trust.
3 Business interests
"Do you own all or part of a business?" Yes reveals the business name and type, "Your ownership percentage", "Estimated value of your share" and how that value was reached, when it started, any shareholder or buy-sell agreement, whether the partner will be involved, and what should happen to the business if the marriage ends. No asks about future plans instead.
4 Debts & income
Debts with rough balances, annual income before tax, other income and any expected changes, such as a career break or a business sale.
5 Your wishes
What should happen to what each brings into the marriage and what they build during it, spousal support, plans for a family home, wishes if one partner dies, other concerns, an optional upload for statements or valuations, and an acknowledgement that the lawyer acts only for them.
Make the template yours
- Relabel the wedding date for postnuptial agreements
- Add the specific disclosure documents you ask for to the upload help text
- Change the currency for clients outside the US
- Add a question about pets, family heirlooms or digital assets if they come up often
- File each questionnaire in that client's folder, never a shared one
- Remove the support question where it's handled differently by local law
Tips for a better prenuptial agreement questionnaire form
- Send the questionnaire well before the wedding — rushed agreements raise questions later
- Use the business section to decide early whether an independent valuation is needed
- Treat the wishes as a starting point for advice, not instructions to draft
- Never share one partner's answers with the other partner's lawyer without permission
Every response becomes a PDF in your vault
The questionnaire is filed as a PDF in the vault folder you choose, with any statements attached, and you get an email. The client's emailed copy is off by default, so it doesn't land in an inbox a partner might see; you can switch it on.
The Responses table shows when each client has finished, and the PDF is ready to bring to your next meeting.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
