What is the power of attorney worksheet form template?
Most of the time in a power of attorney meeting goes on decisions, not drafting: which kind of power, who the attorneys will be, whether they must agree on everything, who steps in if one can't, and what they must never do. Clients who have thought about those questions beforehand get more from the meeting.
This four-page worksheet walks through them. The client gives their details and whether someone is helping, then chooses the kinds of power they want — property and financial affairs, health and welfare, or a general power for a short time — with follow-ups on when a financial power should start and on life-sustaining treatment. They name up to four attorneys, say how they should decide, and list replacements.
The final page captures limits, non-binding wishes, gifts, business or rental property, people to notify, a possible certificate provider, existing documents and questions. A clear acknowledgement says the worksheet has no legal effect. Each one is saved as a PDF in your FileIt vault.

The power of attorney worksheet form, page by page
1 About you
The client enters their full legal name (with a note explaining the term donor, grantor or principal), Date of birth, Phone, Email and Address. If someone is helping, the form asks who and their relationship.
2 Type of power
What kind of power of attorney do you want? offers property and financial affairs, health and personal welfare, a general power for a short time or one task, or not sure. Choosing financial asks when it should start; choosing health asks about life-sustaining treatment decisions. Why are you making it now? is optional.
3 Your attorneys
How many attorneys? (1 to 4) sets how many blocks appear, each with full name, relationship, phone and email. With more than one attorney, How should they make decisions? offers together on every decision, together or alone, a mix, or advice; a mix asks which decisions must be joint. Do you want replacement attorneys? opens a list when the answer is yes.
4 Limits & wishes
The client can set limits or restrictions and non-binding wishes, say whether attorneys can make gifts, and flag a business or rental property. People to notify, a possible certificate provider, any existing power of attorney or will, and questions for the lawyer follow. The acknowledgement confirms the worksheet has no legal effect on its own.
Make the template yours
- Rename the power types to match your jurisdiction, such as lasting, enduring or durable power of attorney
- Remove the life-sustaining treatment question if health powers are made in a separate document where you practise
- Add your fee information in a paragraph
- Add a question about the client's preferred meeting place, such as home visits
- File each worksheet into the client's estate planning folder
- Pair it with an estate planning questionnaire for clients doing both
Tips for a better power of attorney worksheet form
- Talk to the client alone at some point to check the choices are their own
- Ask attorneys whether they're willing before the document is drafted
- Replacement attorneys avoid the power failing if an attorney dies or can't act
- Encourage clients to write wishes down — they help attorneys make choices later
- Remind clients a power of attorney ends on death; their will takes over
Every response becomes a PDF in your vault
Each worksheet is saved as a PDF in the vault folder you choose, and the client can get a copy by email.
The lawyer reviews it before the meeting, discusses each choice with the client, and drafts the document in the form required where they live. The Responses table shows upcoming matters.

Is this power of attorney worksheet free?

Is the worksheet a power of attorney?
No. It's a planning tool. A valid power of attorney must be prepared, signed and sometimes registered under the rules where the person lives.
Can family members fill it in for someone?
Someone can help, and the form asks who. The choices must be the person's own, and the lawyer will confirm that.
Does it work in different countries?
The questions are general. Names and rules for powers of attorney differ, so rename terms and options to match your jurisdiction.
Do clients need a FileIt account?
No. Share the link or send a personal link to each client.
Can I combine it with a will questionnaire?
Yes. Send both links, or add the questions you need from one into the other in the designer.
