What is PS Form 1093?
PS Form 1093 is how you apply for a PO Box at a Post Office. You say whether the box is for personal or business use, give your name, address and contact details, choose one of five box sizes, and list two current IDs: one with a photo and one that proves your physical address. On the back you list everyone who will receive mail at the box and, for a business, the people allowed to pick it up.
The first two pages explain box sizes, fees, renewal payments, keys, refunds and the terms of service. The application itself is pages 3 and 4. An optional section sets up automatic renewal payments by credit card at the Post Office, which is required for the 3-month payment option. USPS also takes PO Box applications online at usps.com/poboxes, except for Business Mail Pickup (Caller) Service and no-fee boxes.
FileIt fills the answers onto USPS's own PDF (August 2019), including the check marks for box size and ID types and the names on the back, and checks that item 3 fits its line. You can sign with FileIt's eSignature or by hand. Then take the whole form and your two IDs to the Post Office; FileIt doesn't submit it.

Common mistakes to avoid
- Bringing a Social Security card, credit card or birth certificate as ID: the form says they aren't accepted.
- Using a non-photo ID that doesn't show your current physical address.
- Forgetting to list everyone who will receive mail at the box.
- Filling in the shaded boxes, which are for the Post Office.
- Choosing the 3-month payment option without setting up automatic renewal.
After you fill it out
Take the whole form and both IDs to the Post Office. Once your details are checked and the fee is paid, you get your box number and keys or combination.
Pay renewals on time: after 10 days of nonpayment USPS closes the box and treats the mail as undeliverable.
Keep your information current with the Post Office. FileIt keeps a copy of your application in your vault.

What ID do I need for a PO Box?
Two current IDs: one with a photo (for example a driver's license, state ID, passport or employee ID) and one that proves your physical address (a lease, mortgage, deed of trust, voter or vehicle registration, or home or vehicle insurance policy).
Can I apply for a PO Box online instead?
Yes, at usps.com/poboxes, paying by card. Online registration isn't available for Business Mail Pickup (Caller) Service or no-fee boxes.
Which box size should I choose?
The form says Size 1 fits 10-15 letter-size envelopes or two rolled magazines. Start with Size 2 if you get more than 15 pieces a week, and consider Size 3, 4 or 5 for magazines and catalogs.
Can my children get mail at my PO Box?
Yes. A parent or guardian can list a minor's name in item 11, and no ID is required for the minor.
How do refunds work if I close the box?
Page 2 lists them: no refund on 3-month payments; on 6-month payments half the fee within the first 3 months; on 12-month payments three-quarters, half or a quarter within the first 3, 6 or 9 months.
Does FileIt rent the box for me?
No. FileIt fills the application so it is ready to hand in. The Post Office checks your IDs, takes the fee and assigns the box.
