What is FK 3084 care allowance application (omvårdnadsbidrag)?

Omvårdnadsbidrag (care allowance) is paid by Försäkringskassan to parents whose child needs more care and supervision than other children of the same age because of a disability. It can be granted from birth until June of the year the child turns 19, at four levels: full, three quarters, half or a quarter. FK 3084 (Ansökan Omvårdnadsbidrag) is the paper application, and the same form is used to ask for a changed split of the allowance between the parents.

You can apply alone or together with the child's other parent. The form asks about each applicant's links abroad, what you are applying for and from which month, how the allowance should be divided, up to three children with their disability and medical certificate, the support the children need, and whether you need an interpreter.

When to use FK 3084 care allowance application (omvårdnadsbidrag)

As soon as your child's needs are clear: the allowance is paid at most three months back from when Försäkringskassan receives the application.
When your child's needs have changed and you want a new assessment.

How to fill out FK 3084 care allowance application (omvårdnadsbidrag), step by step

1 1. You (and the other parent)

Names and personnummer for applicant A and, if you apply together, applicant B, plus three yes/no questions each about living abroad, working abroad and getting benefits from abroad. FileIt prints each personnummer on every page header too.

2 2. What you apply for

A new care allowance, from a month you choose, or a changed split. For a changed split you only fill in 2.a, the interpreter and other information, and the signature.

3 2.a The split

Give each applicant's share in percent. If you apply alone, give the share you want; you get the whole allowance if no other parent gets it for the same child. If you apply together and don't choose, you get half each.

4 3. The children

For up to three children: name, personnummer, the disability, whether you send a medical certificate or one is already on file, and three boxes on living elsewhere, public support such as respite care, and stays abroad.

5 The support the children need

Short answers are enough: treatment and training, structure and planning, practical help, communication, school support and extra supervision. Say which child each part is about, and tick the box if you attach more.

6 4-6. Other information, interpreter, signature

Anything else, whether you need an interpreter and in which language, then the date and phone for each applicant. You sign by hand after printing.

Common mistakes to avoid

Shares in 2.a that don't add up to 100 percent.
Choosing a start month more than three months before the application arrives.
Forgetting the medical certificate, or not saying that Försäkringskassan already has one.
Leaving out applicant B's signature when you apply together.

After you fill it out

Print the form, sign it by hand (both of you if you apply together) and post it to Försäkringskassans inläsningscentral, 839 88 Östersund, with the medical certificate if you are sending one. FileIt doesn't post it for you.
