What is ZUS ZUR parental leave allowance claim?

ZUR (Wniosek o zasiłek macierzyński za okres urlopu rodzicielskiego) is the ZUS application parents use to be paid for parental leave (urlop rodzicielski) when they didn't claim it at the start together with maternity leave. You use it while you are receiving maternity allowance, or after a break, and for each later part of parental leave you take.

On three pages you give your details and your employer's, the parental leave period, why you are claiming (a birth, adoption or foster care, the mother's hospital stay, disability, return to work, abandonment or death, or a discharge from hospital), and the child's details. Page 3 asks who received the allowance before, and whether the other parent will take parental leave allowance at the same time.

Who needs to fill out ZUS ZUR parental leave allowance claim?

Mothers who claimed only maternity leave on ZAM and now want parental leave allowance.
Fathers who take part of the parental leave and claim the allowance for it.
Parents who take parental leave in several parts and claim a further part.
A parent or close family member claiming because the mother is in hospital, has a disability certificate, has returned to work, has abandoned the child or has died.

When to use ZUS ZUR parental leave allowance claim

Before the parental leave period you are claiming, as each part comes up.
When you return to parental leave after a break in the maternity allowance.
If you are claiming maternity and parental leave together from the start, use ZAM instead.

What you need before you start

Your PESEL (or identity document and date of birth), address and your employer's NIP and name.
The start and end dates of the parental leave you are claiming.
The child's name, date and place of birth.
For a further part: the periods of parental leave allowance you and the other parent received before.
The other parent's or family member's PESEL and name, if they received or will receive the allowance.
A Polish bank account number if you want payment into an account.

How to fill out ZUS ZUR parental leave allowance claim, step by step

1 Your details, payer and bank account

As on other ZUS benefit forms: your PESEL, name and address, your payer's NIP and name, and optionally a bank account for the payment.

2 The period and the full-leave statement

Give the start and end of the parental leave you are claiming, printed as dd.mm.yyyy – dd.mm.yyyy. You can also state that you intend to take maternity and parental leave in full, with at least one day of parental leave in the child's first year.

3 Why you are claiming

Pick one reason, from the birth, adoption or foster care through the mother's situation to a discharge from hospital. For a discharge, say whether it was you or the child and give the date.

4 The child

Give each child's name, date and place of birth, and say if a child has the 'Za życiem' certificate.

5 First or further part

Say whether this is the first part of the parental leave allowance or a further one. For a first part, page 3 asks whether you, or the other parent or a family member, received maternity allowance for maternity leave before. For a further part, it asks about earlier parental leave allowance and its dates.

6 Same time as the other parent

Question 3 asks whether the other parent will receive the allowance for parental leave during the same period, and when. Give their details below it.

7 Date and signature

Date page 3 and sign it by hand after printing.

Common mistakes to avoid

Using ZUR when you are claiming maternity leave as well; that goes on ZAM.
Answering question 1 for a further part (or question 2 for a first part); FileIt shows only the one that applies.
Giving an earlier period that overlaps the one you claim now.
Leaving out the other parent's details when you said they received, or will receive, the allowance.

Use ZUR to claim maternity allowance for parental leave on its own, during the maternity allowance or after a break. ZAM is for maternity leave, leave on its terms or supplementary leave, optionally with parental leave in the same claim.

Do I need a new ZUR for each part of parental leave?

The form has a box for a first part and one for a further part, so you fill in a new one for each part you claim.

Can both parents claim at the same time?

The form asks this in question 3: whether the other parent will also receive maternity allowance for parental leave during the same period, and for which dates.

Where do I hand in ZUR?

To your employer if it pays your benefits, otherwise to ZUS.
