What is ZUS ESUN supplementary benefit claim?
ESUN (Wniosek o świadczenie uzupełniające dla osób niezdolnych do samodzielnej egzystencji) is the Social Insurance Institution's (ZUS's) claim form for the supplementary benefit for people who can't live independently. According to the Information sheet attached to the form, you can get it if you are 18 or over, a medical certificate confirms your incapacity for independent living, you live in Poland with Polish citizenship or a right of residence, and your regular public cash benefits together don't exceed a monthly limit that changes every 1 March.
Besides your identity and addresses, the form asks about every regular benefit financed from public funds that you receive or have applied for - a pension, a permanent allowance, a foreign pension - and whether you have also claimed at another pension authority or are in custody. Non-citizens give their right of residence. You then choose how the benefit is paid and how ZUS should reply.
FileIt walks you through each part in plain English, checks your PESEL and bank account number, makes sure each free-text answer fits its box, and prints everything in capitals - with Polish letters intact - into ZUS's own PDF. The finished form is saved in your vault. You print it, sign it and hand it to ZUS yourself.

How to fill out ZUS ESUN supplementary benefit claim, step by step
1 Your details (Dane wnioskodawcy)
PESEL, first name, surname, father's first name (optional), date of birth, sex, citizenship and an optional phone number. The identity document box is only for people with no PESEL.
2 Addresses (Adres zamieszkania, adres do korespondencji)
Your home address, your last address in Poland if you now live abroad or have no home address, and a correspondence address only if ZUS should write elsewhere. For poste restante, a PO box or a pigeonhole, the form explains what goes in which box.
3 Your statement on other benefits (Oświadczenie wnioskodawcy)
Tick whether you have a right to a regular cash benefit financed from public funds, and list up to four in the table. Then say whether you are waiting on another claim, and whether you have or have claimed a benefit from a foreign pension institution.
4 Other information (Inne informacje)
Whether you have also claimed the supplementary benefit at another pension authority, and whether you are in pre-trial detention or serving a prison sentence.
5 Non-citizens (Oświadczenie wnioskodawcy, który nie posiada polskiego obywatelstwa)
Only if you aren't a Polish citizen: your right of residence and its dates, or a permanent right of residence.
6 Payment, attachments, reply (Sposób wypłaty, załączniki, sposób odbioru odpowiedzi)
Choose a bank account in Poland or an address in Poland - you can skip this if ZUS already pays you a benefit, which then sets the method. Give the number of documents attached, say whether you attach the certificate, choose how ZUS replies, date it and sign by hand.
Common mistakes to avoid
- Leaving out a benefit you receive, such as a permanent allowance or a foreign pension - the limit counts them together.
- Listing a benefit without its paying institution or monthly amount.
- Forgetting the document confirming a foreign benefit and its amount.
- For a non-citizen, forgetting the document that grants the right to stay in Poland.
- Typing a bank account number with a wrong digit - FileIt checks it for you.
- Attaching an OL-9 certificate older than a month.
After you fill it out
