What is ZUS ERSU (Mama 4+)?
ERSU is ZUS's claim form for the parental supplementary benefit (rodzicielskie świadczenie uzupełniające), widely known as 'Mama 4+'. You address it to the President of ZUS, who decides whether to grant it.
According to the information printed with the form, the benefit can be granted to a mother who gave birth to and raised, or raised, at least four children, has reached 60 and doesn't have the income needed to live on, and to a father who raised at least four children and has reached 65, after the mother's death, or if she abandoned the children or long stopped raising them. It isn't paid to someone entitled to a pension of at least the minimum pension. You can file the claim at any ZUS office.
FileIt asks the questions in English with the Polish labels, prints your answers in capital letters with Polish letters intact, and fills ZUS's own four-page PDF. You sign it by hand.

How to fill out ZUS ERSU (Mama 4+), step by step
1 Applicant
Give your PESEL number (or an identity document if you have none), first name and surname, and optionally your father's first name, surname at birth and earlier surnames. Then your date of birth, citizenship and, optionally, a phone number.
2 Addresses
Your home address. If you now live abroad, or have no home address, also give your last address in Poland or where you stay. A correspondence address is only for post that should go somewhere else: an address, poste restante, a PO box or a post office pigeonhole.
3 What you claim
You claim the parental supplementary benefit. Say whether you have an established right to an old-age or disability pension, and from which body (ZUS, KRUS, WBE, ZER or another) with its town and your case number, or tick 'not applicable'. Say whether you receive the nursing benefit and, if so, from which body.
4 Payment
Choose a bank account you own or co-own (FileIt checks the 26-digit number) or one of the addresses in Poland given in the claim.
5 Additional information and attachments
Add anything that may affect the decision, give the number of documents you attach, and the date. You sign by hand.

- A mistyped bank account number: FileIt checks the check digits.
- Leaving out form ERU and the documents ZUS lists.
After you fill it out
Print the claim from your FileIt vault and sign it by hand.
File it at any ZUS office with form ERU and the documents you attach. FileIt does not send it to ZUS.
