What is Jamaican passport application (first-time, minor, replacement)?
The Jamaican Passport Application Form is the Passport, Immigration and Citizenship Agency's (PICA) general application. It is used for a first passport, for a child's passport with the consent of a parent or legal guardian, and when a passport has been lost, stolen or is otherwise unavailable. Adults renewing a passport can use PICA's shorter renewal form instead.
The four pages cover your personal data, permanent and mailing addresses, marriage, consent for a minor, your most recent passport, your declaration, two emergency contacts, and an official certification. There are also parts for applicants who must wear headgear for religious reasons and for applicants born outside Jamaica, plus space for supplementary information. Two more pages hold PICA's guidelines on documents, photographs, certification, consent for minors and lost passports.
FileIt explains each part, pre-fills the applicant's name, date of birth and email from the People in your FileIt vault, fills in the minor-consent and certification lines from your answers, wraps longer statements onto the form's lines, and prints everything into PICA's own PDF. The finished form is saved in your vault, ready to be signed and certified by hand.

Common mistakes to avoid
- Using an official who is a relative, or who has known you for less than 12 months, to certify the form.
- Forgetting that the official must also certify the back of one photograph, with the wording given in the guidelines.
- Signing outside the signature box on page 2.
- Sending photocopied or laminated documents.
- Photographs that are older than 6 months, have an embossed seal, or show head covering without section H being completed.
- Not including a police report for a lost or stolen passport.
After you fill it out
Print the finished PDF from your FileIt vault. Sign inside the box on page 2 and on the declaration line (a parent or guardian signs section C for a child).
Have the application and one photograph certified by an eligible official (section G). In Jamaica the guidelines say no fee should be paid for certifying an application.
Take the form, photographs and original documents to PICA, or overseas to the Jamaican embassy, high commission or consulate. FileIt does not submit anything for you.
When the new passport arrives, add it to your FileIt vault so its expiry date is tracked and you are reminded before it runs out.
