What is INSS 6-063 - review of permanent disability?

In Spain, a permanent disability (incapacidad permanente) recognised by the Instituto Nacional de la Seguridad Social (INSS) can be reviewed if the person's condition gets worse, improves, or turns out to have been misdiagnosed. Model 6-063 is the request the person concerned — or someone acting for them — files to start that review.

The form is short: the applicant's details; the details of a person providing support measures for a person with disabilities (a de facto or court-appointed guardian, curator or court-appointed defender) or of a representative, if one files it; the reason for the review; your allegations; and two consents. The last two pages are explanatory and data-protection notes.

1 1.1 The applicant

Surnames and name, DNI / NIE / passport, home address with number, block, staircase, floor and door, postcode, town, province, country, PO box, nationality, Social Security number, phones and e-mail. Your name and ID are repeated at the top of page 2.

2 1.2 Support measures

Only if the request is filed by a person providing support measures for a person with disabilities: a de facto guardian, court-appointed de facto guardian, curator or court-appointed defender, with their details. A public body acting in that role also gives the position held, its name and its tax number.

3 1.3 Representative

Only if a representative or an attorney registered in the electronic register of powers of attorney files it, with their details.

4 2. Reason for the review

Tick worsening, improvement or diagnostic error.

5 3. Allegations

Explain what has changed since the decision and list the reports you enclose. The box holds about eight lines.

6 Consents and signature

Say whether the INSS may obtain the data and documents it needs from other administrations (if not, you must supply them within 10 working days) and whether it may contact you by mobile and e-mail. Then the place and date, your signature and the INSS provincial office.
