PPTC 054 — Adult Simplified Renewal Passport Application

What is PPTC 054?

PPTC 054 is the Adult Simplified Renewal Passport Application from the Passport Program of Immigration, Refugees and Citizenship Canada (IRCC). If your most recent Canadian passport meets a short list of conditions, this form lets you renew it without a guarantor, without sending proof of citizenship and without a signed photo. The current edition is PPTC 054 (02-2026) E; the French version is PPTC 055.

The trade-off is that the renewal relies on your previous passport. Section 1 has to match that passport exactly — same name, same order, same date and place of birth, same sex identifier — or the application is rejected and returned. If anything has changed, or your old passport does not qualify, you use the full adult form PPTC 153 instead.

Who needs to fill out PPTC 054?

• Canadian adults renewing a passport that was issued in the last 15 years.
• People whose previous passport was valid for 5 or 10 years (not a short-validity or temporary document).
• Applicants whose previous passport was issued when they were 16 or older — a passport issued as a child cannot be renewed this way.
• Those who want the same name, date of birth, place of birth and gender identifier as on their most recent passport (or its observation sticker).
• Canadians living in the USA renewing by mail under the same conditions.
• If you changed your name, want a different sex identifier, or cannot give your last passport's number and dates, use PPTC 153 instead.

When to use PPTC 054

• When your adult passport is nearing its expiry date, leaving time before any planned trip — the form advises not finalizing travel until the new passport arrives.
• Up to 15 years after your most recent passport was issued; after that you must reapply with PPTC 153.
• If your current passport still has more than 12 months left, include a written explanation of why you are renewing early, or the application may be refused.
• Both pages must be signed within the 12 months before the Passport Program receives the form.

What you need before you start

• Your most recent passport, including its number, date of issue and date of expiry, plus any other unexpired Canadian passport issued to you.
• Two identical, unaltered photos taken by a commercial photographer within the last six months (no guarantor signature needed).
• Your surname at birth and any other former surnames, and one or both parents' surnames at their birth.
• Your current home address and, if different, a mailing address for delivery.
• Your addresses for the last two years and your jobs, schooling or other activity for the same period, with months and years.
• Contact details for two references who are not relatives and have known you for at least two years.
• An emergency contact's name, relationship, phones and address.
• Form PPTC 203 if your most recent passport is still valid but lost, stolen, damaged or found.
• The fee for the 5- or 10-year passport (the Passport Program publishes current fees online).

How to fill out PPTC 054, step by step

1 Section 1: Personal information

Write your surname and given names exactly as they appear on page 2 of your most recent passport, spelled the same and in the same order. If your name has changed, this form cannot be used. List every former surname, including your surname at birth, and the surname of one or both parents at their birth (or UNKNOWN).

Give your place of birth, and your date of birth and sex exactly as they appear in the previous passport, then your eye colour, height, home address, optional mailing address (where the passport is delivered), email and phones. Sign within the border on page 1, with the date and place you sign.

2 Section 2: Most recent Canadian passport

Enter your most recent passport's number, date of issue and date of expiry. These three details are the heart of the renewal: if you cannot provide them, you cannot use this form. Enclose that passport and any other unexpired Canadian passport issued to you.

Tick whether you want the old passport returned or securely destroyed. If neither box is ticked, it is destroyed, so check for visas you still need first. A passport ever reported lost or stolen, or kept because it is damaged, is not returned.

3 Section 3: Period of validity

Choose a 5-year or 10-year passport. The form does not print the fees; they are adjusted every year and listed on the Passport Program's fees page.

4 Section 4: Additional personal information

Part A asks whether you have lived at your current home address for the last two years; if not, list each earlier address with country and the months you lived there. Part B asks whether you were employed, in school or doing something else (such as homemaking, unemployed or retired) and for the name, address, phone, field and dates of each employer or school. Use form PPTC 056 if you need more space.

5 Section 5: References

Name two people who are not your relatives, are 18 or over and have known you for at least two years, with their address, phones, email and years known. Unlike PPTC 153, there is no guarantor on this form, so the references are the people the Passport Program may contact to confirm your identity — let them know first.

6 Section 6: Emergency contact information

Mandatory. Give the name, relationship, phones, email and address of someone to contact if you need emergency help while outside Canada. Then sign and date page 2.

7 Credit card information (section D, mail-in only)

If you mail the application and pay by Visa, MasterCard or American Express, fill in the card type, name on the card, your name and the exact amount in Canadian dollars. Common mistakes to avoid

• Writing your name differently from your last passport, for example adding a middle name or changing the order. Section 1 must match exactly or the form is rejected and returned.
• Using the renewal form after a name change or to change your sex identifier — both require PPTC 153.
• Trying to renew a passport issued when you were under 16, or one older than 15 years.
• Leaving the previous passport's number, issue date or expiry date blank.
• Forgetting to enclose the old passport, or not adding PPTC 203 when it is lost, stolen or damaged but still valid.
• Signing only one page — both pages need your usual signature.
• Choosing relatives as references.

After you fill it out

Print the PDF and sign both pages in black or dark blue ink (the box on page 1 and the line on page 2). No guarantor is needed and the photos do not need to be signed.

Submit it yourself: in person at a Passport Program office or participating Service Canada Centre, or by mail to the Passport Program address in section F of the instructions, which depends on whether you are in British Columbia or the territories, another province, or the USA (a courier address is also given). Enclose your photos, your most recent passport and the fee. The form recommends a trackable mail service.

Keep the filled PDF in your FileIt vault. When the new passport arrives, update the passport record with the new number and expiry date so FileIt's reminders are set for the next renewal.
