What is HLTH 115 MSP baby enrolment?
HLTH 115 is the paper form a parent who is already enrolled in British Columbia's Medical Services Plan (MSP) uses to enrol a new baby. It asks for the parent's name, address, date of birth, Personal Health Number (PHN) and phone number, then the baby's legal names, where the baby was born, gender, date of birth and, for an adopted baby, the adoption date.
The form says it must be completed and returned within 60 days of the baby's birth. Once it is processed, a BC Services Card is issued for the baby. Residents of BC are required by law to enrol themselves and their dependants with MSP.

Who needs to fill out HLTH 115 MSP baby enrolment?
- Parents in British Columbia who are enrolled in MSP and have just had a baby.
- Parents who have adopted a baby, or whose adoption is in progress.
- Parents whose baby was born at home, who attach a copy of the birth certificate or Certificate of Live Birth.

1 1 Parent information
Your legal last, first and second names, your address (apartment, street number, street name and city, province and postal code), your date of birth, your Personal Health Number and a daytime phone number. Every box on the form is a row of letter cells, so the names have room for 18 (last) and 12 (first and second) letters, and the street name and city share one box of 32.
2 2 Newborn information
The baby's legal names, the hospital's name and city, gender, date of birth and, only if the baby was adopted, the adoption date. For a home birth, leave the hospital blank and attach the birth certificate or Certificate of Live Birth.
3 3 How to enrol your baby
Information only. If you are enrolled on a self-administered, group or FNHA account, send the form to Health Insurance BC. If you are enrolled through SDPR, take it to SDPR.
4 4 Sign and date the declaration
You declare that the information is true, that the child is a resident of British Columbia and that you agree to MSP's terms, then sign and date the form. FileIt fills in the date; you sign in the signature area.

Common mistakes to avoid
- Sending it after the 60 days the form asks for.
- Using a nickname instead of the legal names, or a PHN that is not yours (it is the 10-digit number starting with 9 on your BC Services Card).

- Forgetting the birth certificate copy for a home birth, or the adoption papers for an adopted baby.
- Not signing the declaration.
