What is Deceased person's super claim cover sheet (NAT 75530)?

The ATO holds some superannuation money itself, such as unclaimed super and super guarantee amounts. When the account holder has died, the paper claim is the Application for payment of ATO-held superannuation money (NAT 74880), and the ATO asks you to include this cover sheet with it.

The cover sheet identifies the account holder: their tax file number or, if you can't give it, their title, family name, given names and date of birth. There is no signature on it.

FileIt fills in the ATO's own cover sheet and checks the TFN check digits. You print it and put it on top of your completed NAT 74880 and the supporting documents.
