What is Verf 15 — tax registration questionnaire (companies)?

Verf 15 is the Austrian tax office's registration questionnaire for corporations founded under Austrian law (nach inländischem Recht gegründete Körperschaften), such as a GmbH or an AG. It is published by the Federal Ministry of Finance (Bundesministerium für Finanzen, BMF). The form itself says associations use Verf 15a and private foundations Verf 15b instead. The edition covered here is Verf 15-PDF, Version vom 24.01.2025.

The questionnaire asks for the company name as in the companies register, the date of the opening, the start of trading or the first payment into the share capital, the register number, the business year, the place of management, phone and fax, an exact description of the activity and any branches, the shareholders of a GmbH with their tax office and tax number, the board members or managing directors, the expected turnover and income for the opening year and the following year, the small-business VAT option, other levies and any representative.

FileIt explains each question in English, with the German terms alongside, and prints your answers into the official BMF PDF, with dates and amounts in Austrian format. The finished PDF is saved in your FileIt vault. FileIt doesn't send anything: the company signs the questionnaire and sends it to the tax office with the documents asked for.

How to fill out Verf 15 — tax registration questionnaire (companies), step by step

1 Page 1: the cover sheet

Page 1 is normally the tax office's covering letter. If you send the questionnaire on your own, fill in the office's address and the company's name and address, and tick the documents you enclose. FileIt leaves the office's own boxes (letter date, contact, further documents) empty.

2 The company

The company name, the start date with which event it is (1) opening, (2) start of trading or (3) first payment into the share capital, the companies register entry and number, the business year, the § 20 application, the expected number of employees, the place of management, phone and fax, the activity and any branches.

3 Shareholders and directors

For a GmbH, list each shareholder on one line with name, date of birth and address, then their tax office and tax number. Below, list the board members or managing directors with name, date of birth and address and, for a GmbH, their share in percent. Further people go on a separate sheet.

4 Turnover and income

The expected annual turnover and income from business for the opening year and the following year, in euros. A loss is entered with a minus sign.

5 VAT and other levies

Small businesses (annual turnover up to 55,000 euros, § 6 (1) Z 27 UStG 1994) say whether they intend to apply for normal VAT taxation. The form notes that companies with only VAT-exempt turnover without input tax deduction attach form U 15 if they need a UID number; all others are given one automatically. Then tick any chamber levy, motor vehicle tax, NoVA, advertising tax or other levy.

6 Representative and signature

If a representative acts for the company, give their name and address and say whether a copy of the power of attorney is enclosed or they rely on their authorisation. The representative can confirm the directors' identity. The company confirms the answers are complete and correct, and signs and dates the form.

Common mistakes to avoid

  • Using Verf 15 for an association, a private foundation or a partnership. They have their own questionnaires.
  • Describing the activity too vaguely. The form's own example asks for 'Gemüsehandel' rather than 'trade in goods of all kinds'.
  • Listing shareholders for an AG. The shareholder rows are only for a GmbH.
  • Leaving out the documents the cover page asks for, such as the articles of association or the opening balance sheet.
  • Forgetting form U 15 when the company has only VAT-exempt turnover and needs a UID number for intra-EU trade.

After you fill it out

Print the questionnaire from FileIt, have it signed for the company, and send it to the tax office with the documents asked for. The office's cover page asks for it back within one month.

The office uses the answers to register the company and give it a tax number, and a UID number where that is issued automatically. Keep the filled questionnaire in your FileIt vault with the company's founding papers.
