What is a veterinary treatment authorization?

Vets need someone who can consent to treatment and promise to pay. When the owner is away and unreachable, a sitter or kennel without written authority can be left standing in the waiting room. This form gives them that authority, with limits you set before you leave.

It names the owner, the carer and the dates, with the owner's phone and a backup contact. It records the pet's name, species, breed, age, microchip and any conditions, medication or allergies, plus the usual vet and pet insurance. You choose how much the carer may agree to, a spending limit when you can't be reached, and your wishes on resuscitation and euthanasia. It covers how bills are paid, authorises the vet to talk to the carer, and can be cancelled at any time.

Using the veterinary treatment authorization, step by step

1 You and the carer

Your name fills in from your account. Add your phone while away, a backup contact, the carer and the dates the authorization covers.

2 Your pet

Enter your pet's details and any health conditions, medication or allergies, plus your usual vet and insurance policy.

3 Your limits

Choose what the carer may agree to, set a spending limit, and decide your resuscitation and euthanasia wishes. They're hard choices, best made calmly now.

4 Sign before you go

Sign, send it to the carer and send a copy to your vet. Everyone has the same signed PDF, stored in your FileIt vault with your pet's records.

Tips before you send it

  • Give your vet a copy before you leave, and ask if they'd like card details on file.
  • Set the spending limit at what you'd really pay without a phone call.
  • Leave medication, the insurance policy number and the vet's address with the carer.
  • Use it with a pet sitting agreement, which covers the day-to-day care.

Veterinary treatment authorization: frequently asked questions

Will every vet accept it?

Most vets welcome written authority, but a vet keeps their own judgement and may ask the carer to sign the practice's consent form at the visit.

What if treatment costs more than the limit?

The carer needs your agreement, except for treatment the vet says can't wait to relieve pain or save your pet's life.

Do I have to decide about euthanasia?

You choose between letting the carer agree on the vet's advice, or keeping your pet comfortable while they keep trying to reach you.

Who pays the vet?

You do. Choose whether the vet bills you directly, charges your card on file, or the carer pays and you repay them within 7 days.

Can I cancel it early?

Yes. Tell the carer and your vet; the authorization stops then.
