What is a tattoo and piercing consent?
Every tattoo and piercing studio needs a signed consent from each client: a record that they are old enough, that they have disclosed health conditions that matter, that they approved the design and placement, and that they understood the risks and the aftercare. Many local licensing rules expect you to keep these records.
This form records the studio, the artist and any licence number; the client's name, date of birth and the ID you checked; the service, design, placement, jewellery and price. The client confirms they've approved the stencil or placement, are an adult of the required age, and declares relevant health conditions. It explains the risks fairly, sets out your aftercare instructions and touch-up policy, covers record photos, and does not try to exclude liability for your own negligence. Using the tattoo and piercing consent, step by step
1 Studio and artist
Your studio name fills in from your account. Add the artist or piercer and, if you're licensed, your licence number.
2 Check ID and age
Enter the client's name and date of birth, and record the ID you checked. Check the minimum age for the work where you are.
3 Describe the work
Choose tattoo, piercing or cover-up, and describe the design, placement, jewellery and price. The client confirms they approved the final stencil or mark.
4 Aftercare and signing
Edit the default aftercare to match your own instructions and add your touch-up policy. Send it to the client's phone at the counter or before the appointment; they sign and initial, you countersign, and the record is kept in your FileIt vault.
Tips before you send it
- Send the form before the appointment so the client can read it calmly, then confirm the health answers in person.
- Keep consent records for as long as your licensing rules require.
- Give the client a written or printed copy of the aftercare as well as the signed form.
- If the client discloses a condition you're unsure about, pause and suggest they check with a doctor.

Can I use this for a client under 18?
This form is written for adult clients. Whether minors can be tattooed or pierced at all, and with what parental consent, depends on local law — check your local rules and adapt your process.
Does the client waive all claims?
No. The client accepts the inherent risks described, but the form says it does not limit your responsibility for your own negligence or rights that can't be excluded by law.
