What is Form 1194 — Certification of Immigration Status (DASP)?

When you leave Australia after working on a temporary resident visa, you can claim your super back as a Departing Australia Superannuation Payment (DASP). If you claim on paper directly with your super fund and your withdrawal is AUD5,000 or more, the fund needs a Certification of Immigration Status from the Department of Home Affairs. Form 1194 is how you ask for it.

The certification confirms that your temporary resident visa is no longer in effect and that you've left Australia. The Department emails it straight to each super fund you name and sends you a copy by email or post. There's a fee, paid in advance through ImmiAccount, and each person needs their own form. If you use the ATO's DASP online service instead, your immigration status is checked online and you don't need this form.

FileIt fills the current Form 1194 (design date 03/21) in the Department's own PDF, prints names and addresses with accents correctly, shows only the questions that apply to you, and keeps the finished form in your FileIt vault. You sign it, attach your receipt and evidence, and email it to the Department yourself.

Who needs to fill out Form 1194 — Certification of Immigration Status (DASP)?

  • Former temporary visa holders who have left Australia and are claiming DASP on paper from their super fund.
  • Anyone whose fund asks for a Certification of Immigration Status because the payout is AUD5,000 or more.
  • People who arrived as children and need to give a parent's details because they no longer hold their first passport.

When to use Form 1194 — Certification of Immigration Status (DASP)

  • After you've left Australia and every temporary resident visa you held has expired or been cancelled.
  • When you lodge a paper DASP application with your super fund for AUD5,000 or more.
  • Not when you claim through DASP online — the ATO checks your immigration status there.
  • Not for a claim under AUD5,000 — your own evidence, such as your visa grant letter and passport stamps, is usually enough.

What you need before you start

  • Your current name, the name in the travel document you entered Australia with, and any other names, with certified copies of name-change evidence.
  • Your date of birth, countries of birth and citizenship, correspondence address and phone numbers.
  • Your first arrival and last departure: dates, places, flight numbers if known, and the passport numbers you used.
  • Each super fund's name, postal address and email address.
  • The receipt number for the fee, paid through My Payments in ImmiAccount, and a printed copy of the receipt.
  • If you arrived as a child and no longer hold that passport: your parents' names and dates of birth.

How to fill out Form 1194 — Certification of Immigration Status (DASP), step by step

1 Questions 1–3: your names

Give your full name, your name as it appeared in the travel document you entered Australia with (or tick that it's the same), and any other names you've been known by, with the reason for each change.

2 Questions 4–11: about you

Your sex, date of birth, countries of birth and citizenship, whether you've left Australia, your correspondence address, phone numbers, and whether the Department may contact you by email or fax.

3 Questions 12–14: travel

Your first arrival on the temporary resident visa and your last departure. If you came as a child and no longer have the passport you arrived on, switch on question 14 for your parents' details.

4 Question 15: your super funds

Each fund's name, address and email — the Department sends the certification directly to the fund. A second fund can be added; for more, list them in your email.

5 Questions 16–21: help, contact, payment

Say whether someone helped you fill in the form, where written communications should go, give your ImmiAccount payment receipt number, then sign and date the declaration consenting to the certification going to your funds.

Common mistakes to avoid

  • Sending the ATO's DASP claim form to Home Affairs — that goes to your fund or the ATO, not the Department.
  • Forgetting to pay the fee first in ImmiAccount and attach the receipt.
  • Leaving out the fund's email address — the certification is sent to funds electronically.
  • Not attaching certified copies of evidence for a name change.
  • Applying before every temporary resident visa has ceased or before you've left Australia.

After you fill it out

Sign and date the declaration on page 5. Print from FileIt and sign by hand, or send the PDF for e-signature.

Email the form, your payment receipt and any evidence to the address printed on the form, [email protected]. FileIt doesn't send it.

Once the Department finalises your request it emails the certification to your super fund(s). Keep a copy of everything in your FileIt vault.

Form 1194 — Certification of Immigration Status (DASP): frequently asked questions

Do I need Form 1194 to claim DASP?

Only for a paper claim lodged with your super fund where the withdrawal benefit is AUD5,000 or more. DASP online checks your status for you, and for smaller paper claims your own evidence is usually enough.

Is there a fee?

Yes. You pay it through My Payments in ImmiAccount (Pre-Pay Paper Service) before you send the form, and attach a copy of the receipt. The form says it isn't generally refunded if the request isn't successful.

Where does the certification go?

The Department emails it directly to each super fund you list at question 15 and sends you a copy by email or post.

Can my partner and I use one form?

No. Each person who wants a Certification of Immigration Status completes a separate Form 1194.

What if I arrived in Australia as a child?

If you no longer hold the passport or travel document you used on your first arrival, give your parents' names, former names and dates of birth at question 14.
