What is Form 1022 — Notification of changes in circumstances?

Section 104 of the Migration Act 1958 requires visa applicants to tell the Department of Home Affairs about any change in their circumstances that affects an answer on their application form — a new address, a new passport, a marriage, separation or birth in the family, a new job. Form 1022, Notification of changes in circumstances, is the paper form for doing that.

The duty lasts until your visa is granted if you applied in Australia, or until you are immigration cleared if you applied outside Australia. The form warns that not complying can lead to a visa being cancelled, and that the information you give is taken into account when your application is assessed. If an answer was wrong when you gave it, rather than changed since, the form points you to form 1023 Notification of incorrect answer(s) instead.

Who needs to fill out Form 1022 — Notification of changes in circumstances?

Anyone with a visa application in progress whose address, contact details, passport, relationship status or family has changed.
Partner, family and skilled visa applicants whose household changes during processing — for example a birth, marriage or separation.
Applicants whose matter is being considered for another reason, such as a proposed visa cancellation, who need to update their details.

When to use Form 1022 — Notification of changes in circumstances

As soon as something changes that affects an answer you gave on your application form.
Not after your visa has been granted (if you applied in Australia), or after you have been immigration cleared (if you applied outside Australia) — the form says you don't have to notify changes after those points.
Use form 1023 instead if you need to correct information that was wrong when you gave it.

Your full name, date and place of birth, relationship status and citizenship.
Your passport number and country, and any national identity number.
Your current residential address (not a PO box), correspondence address and phone numbers.
Your client number or file number, if the Department has given you one.
Names, dates of birth, passport details and addresses of your partner and dependants included in the application.
The old information from your application and the new correct details for each change.
Visa and application details if you know them: date granted, stay period and visa class, or date lodged, where lodged and the class applied for.

How to fill out Form 1022 — Notification of changes in circumstances, step by step

1 Your details (questions 1 to 12)

Give your name, date and place of birth, relationship status, passport and identity card details and citizenship, then your residential and correspondence addresses and phone numbers. Question 11 asks whether you agree to the Department communicating with you by fax, email or other electronic means; if yes, give your email address. Question 12 is your client or file number, if known.

2 Details of other applicants (question 13)

If your partner (spouse or de facto partner) or any dependants are or were included in your application, give each person's name, date of birth, country of birth, passport number and country, and residential address. There is room for three people; attach additional details for more.

3 Details of changes (question 14)

For each change, write the information on your application that is no longer current, then the new correct details. There is room for three changes on the form — be specific and include the date each change happened.

4 Visa and application details (questions 15 and 16)

If you know them, give the visa's grant date, stay period and class, or the application's lodgement date, where it was lodged and the visa class applied for. These help the Department match the form to your case.

5 Declaration (question 17)

You declare that the information is true and correct, that you have read form 1442i Privacy notice, and that you understand how the Department may use your information. You sign and date it, and each other applicant aged 18 or over signs and dates their own box.

Common mistakes to avoid

Using a post office box as your residential address — the form says it is not acceptable.
Describing only the new situation without saying what it replaces on your application.
Using Form 1022 to correct an answer that was wrong from the start — that is form 1023.
Forgetting the signatures of other applicants aged 18 or over.
Leaving questions blank — the form says you must answer all questions.

After you fill it out

Sign and date the declaration, and have each other applicant aged 18 or over sign theirs.

Return the completed form to the office where you lodged your application or, for any other matter such as a proposed visa cancellation, to the office of the Department responsible for it. If you're unsure which office is responsible, the form says it can be submitted to the nearest office of the Department.
